Maggi Leise, P.T. received her degree in physical therapy from University of Illinois at Chicago in 2000. After graduation, she began her career in an Olympic weightlifting facility prior relocating to Chicagoland to focus on manual therapy techniques. Maggi spent over a decade designing and evolving a women-specific physical therapy program that is now nationally utilized.
Maggi has a passion for athletics and performing arts, and she is driven to empower her athletes and patients to dream big, make a plan, and follow-through. She believes in self-discipline, learning from setbacks, and being held accountable along the journey of Life. Maggi has an unusual sense of humor and believes laughter really is good for the soul. She is lucky to have her husband, Greg, and 4 beautifully ornery daughters to keep her on her toes.
Physical Therapist Assistant
Mike joined the Prism Performance and Physical Therapy team in November of 2017. Prior to joining the team he graduated from Andrew High School in 2006. He began his journey at Western Illinois University and went on to graduate from the 2011 Fox College Physical Therapist Assistant program in Tinley Park becoming a licensed PTA. Since then he has been focused on learning and working with patients and co-workers to better himself as well as making a positive influence on the people surrounding him.

Caroline is a Southern Illinois native from Marion, Illinois. She graduated from Southern Illinois University of Carbondale receiving her Bachelor’s of Science in Exercise Science. She then continued her studies at Fox College through their Physical Therapist Assistant program, where she became a licensed clinician. As a clinician, Caroline strives to dedicate her skills and passion to make a positive impact on the lives of our patients and their families.
Nicky joined the Prism Performance and Physical Therapy team in June of 2023. She has a successful business that she runs alongside her husband since 2002. When Maggi approached her to come and work as her Office Manager, she jumped at the chance to be a part of a passionate group of people. She is excited to be a part of the vision, passion and empowerment that Prism stands for.
Have joined the Prism team back in 2021 as a Performance Coach and now has been added on as a Rehab Technician this past year. She is currently a Senior at Lockport Township High School and is planning on focusing her studies in Special Education after she graduates. Haven loves helping the patients and keeping the therapists organized.
Paige is currently a Junior at Lockport Township High School where she is a member on the Dance Team as well as Competitive dancer at her studio. Paige has been a long-time patient of Prism Performance and Physical Therapy and when she was offered to join their team she was excited for the opportunity. Being an athlete and patient Paige is excited to contribute her experience to benefit to the health and well-being of others.
